Older adults require specialized care, because the human body changes in so many ways with age. Geriatric assessments can take up to two hours, and each domain is evaluated very closely. This includes a comprehensive internal medicine review, followed by a review of the following:
Cognition
Physical health
Mobility
Medication review
Goals of care
Interdisciplinary Model
We believe in an interdisciplinary model of care to ensure that our patients receive the highest quality and most comprehensive care possible. Our clinics are supported by the following members:
Registered Nurse (with dedicated training and experience in geriatrics)
Support Counselor (from the Alzheimer's Society of Grey Bruce)
Geriatrician (board-certified physician who specializes in older adults)
Others (reached out on an as-needed basis)
- may include physiotherapist, occupational therapist, dietitian, pharmacist, or other specialist physicians
Our Program
Our current geriatric program is based out of the Owen Sound hospital, and is continuing to expand. It currently includes the following:
Inpatient Geriatric Consult Service
This includes support to five hospitals in the Grey Bruce region.
Comprehensive Geriatric Clinics
Memory Clinics
Geriatric Resource Nurses
Patients are assessed in their home by a registered nurse, and then the care plan is reviewed by a geriatrician
Behavioural Supports Ontario (BSO)
Patients are assessed in the community by BSO team members, and cases are then reviewed with a geriatrician or a geriatric psychiatrist
